Transcript: HCM 520-QI Presentation Introduction Overview Medication Error Case Overview Root Cause Analysis Employee's Roles in Error Recommendations for Improvement Facility Assessment and Improvement Plan A 16 year-old female was admitted due to pain as a result of non-specific colitis. A Broviac line was inserted after a peripherally-inserted line failed. The tubing on the line ultimately became clogged and inoperative. The patient was returned to surgery and the clogged line was replaced. It was determined that the particulate matter in the solution was incorrect with too much calcium and phosphorus. Detailed Case Review ACME Hospital TPN Error TPN Error at ACME Hospital Timeline Timeline Friday Case Review of TPN Error Later that Saturday Saturday Sunday Patient admitted Friday for pain resulting from non-specific colitis. A peripherally-placed line was unable to be inserted. On Saturday, a Broviac catheter was inserted during surgery to allow for administration of total parenteral nutrition support. After the surgery, a nurse noticed that the line continued to be clogged despite attempts to clear it. The patient was then returned to surgery for insertion of a replacement line. The following day, the TPN bag was examined and it was found that the solution had a precipitate that had settled to the bottom of the bag as the calcium and phosphate in the solution caused the line to clog because they were higher than established norms for the solution. Root Cause Analysis RCA Questions to be asked: The patient's TPN line was clogged due to particulate material from a higher-than-normal calcium and phosphate solution. This led to a second surgery for a replacement line to be placed. The patient did not require any follow up surgeries and the rest of her care was uneventful. TPN Event Description of the Event Upon review of the procedures that occurred in the case, it was found that several potential places to intervene were missed by both the physician and pharmacists. The physician who ordered this solution did not have a nutritionist on staff to consult with and then ordered a solution with a higher level of calcium than normal. The pharmacists who reviewed and entered the ordered failed to recognize the error, as did the vendor pharmacist whose role it was to check the composition of electrolytes. The solution was sent to the hospital, scanned in and used for the patient's surgery. Reasons for the event Why Did this Event Occur? The usual checks and balances within the system failed to pick up on the discrepancy in the order, as the various levels of pharmacists did not notice that the calcium level was too high. Likewise, the nutritionist wasn't at work on the weekend per hospital staffing schedules and was thus unable to review the order and ensure its accuracy. There also appeared to be no alert from the computerized medical systems at both the hospital and outside vendor, letting personnel know that levels were incorrect. Overall, communication between the various levels of professionals was inadequate. Ordering physician- the physician ordered a solution with a higher-than-normal level of calcium and phosphorous, which caused the line to clog. This physician did not have the back-up oversight of a nutritionist to check levels. Pediatric pharmacist- Reviewed the TPN order and failed to see the discrepancy. IV Room Pharmacists- At both levels, these pharmacists also failed to see the discrepancy and incorrectly check electrolyte balances. Vendor pharmacist- this pharmacist also failed to see the error and sent the solution as ordered to the hospital. Surgical team-did not see the error at time of surgery QI Recommendations Recommendations Due to the fact that the pharmacists all filled the order based upon what was written by the physician, rather than an error in prescribing what was written, the first recommendation is to engage ACME Hospital in changing staffing levels so that a nutritionist is on staff over the weekend. If this is impossible due to financial restraints, it is recommended that a policy be added wherein all orders for TPN be reviewed by the on-call nutritionist to ensure accuracy. TPN Is considered to be a "high-alert" medication as it has a risk of significant harm if incorrectly utilized, (Boullata, 2013). Ingredients and utilization process can lead to errors, including the prescribing, preparation and dispensing. The process of TPN involves many steps and numerous personnel and organizations and this can lead to a higher likelihood of errors.
