This small Device, which is created in excel, has created satisfaction towards the medical department. Many facilities still audit nursing and physician documentation by using paper forms. Hospitals also use Excel Dashboards that is created by non-technical administrators to help monitor health care activities. The EMR stands for Electronic Medical Records. This is a online Data base that keeps up with the records of patients that come in and out of medical facilities. Instead of paper medical records employees can now use computers. These reports help them to keep track of their medical insurance records; Insurance coverage reports are beneficial because this helps many people to keep track of where they are financially. Weed described a system to automate and reorganize patient medical records to enhance their utilization and thereby lead to improved patient care. In 1970, the POMR was used in a medical ward of the Medical Center Hospital of Vermont for the first time. At this time, touchscreen technology had been incorporated into data entry procedures Electronic health records Today, in the United States, we all live in an age of technology and science. The use of technology and science has revolutionized our way of life. There are few things in history that have influenced our lives more than a computer. Today, there cannot be any field that is absent of the influence of computer applications. From farming to rocket science, computers have a huge role to play. The use of the computer has been on the increase for some time in many fields. Medicine is one of the many fields that have made tremendous strides in the twentieth century due to the advent of computers. Electronic health records (EHRs) are changing medical assistants' jobs. More and more physicians are adopting EHRs, moving all their patient information online. Assistants need to learn the EHR software that their office uses. Medical assistants take and record patients’ personal information. They must be able to keep that information confidential and discuss it only with other medical personnel who are involved in treating the patient. What do they Do? Transcript: The system identifies real time issues of patients in handling their medical records and provides a suitable solution. The system simplifies the functioning by digitally storing information pertaining to patients and their ailments. The system identifies two users Doctor and Patient. The role of the doctor is to register the patient, treat him for his ailment. The doctor is provided with a facility to resume treating a patient on his existing ailment or create a new one. The doctor can also view the test reports for an ailment as uploaded by the patient post receiving the reports from the diagnostics lab. Processor : Intel Pentium (PIV 2.2Ghz or higher) Hard Disk : 40 GB hard disk space Ram : 1 Gb Ram Operating System : Windows 32 Bit or higher Programming Environment : Java Database : Oracle Web server : Apache Tomcat Web browsers : Internet Explorer Scripting language : Java Script Database Drivers : Jdbc/Odbc Drivers(Type 1) The module allows the doctors to view the registered patients. A tabulated listing of the patient with the registration details are shown. This module allows the patient to login and view his prescription. The module collects all his existing ailments and prompts him to choose the ailment for which he wants to see his prescription. View Patients Thank You! This module allows the doctor to view reports uploaded by the patient for a particular ailment. The images of the reports are shown to the doctor. Upload Reports The administrator is the core user of the application who maintains other users. View Reports Proposed System: In the proposed system automation of the treatment and having it across a centralized database makes the system easier to handle the documents and maintain them. The patient registers in a hospital and is provided with a unique ID. The hospital uploads the patients prescription and details the test. The diagnostic lab can view the tests to be conducted and perform the test. The pharmacist can view the prescription and issue the medicines. Every document of the patient is associated with his unique id. Irrespective of the hospital the patient goes his records are visible to the doctor of the new hospital. ADMIN PATIENT This module helps the doctor to choose the consulting patient and treat him. The module prompts the doctor needing him to identify the patient as an existing or new patient. For existing patients the ailments are shown and the previous history can be seen. The doctor can proceed to update the prescription. In the case of a new ailment the details are saved into the database The users use the application to register the patient and treat him. EXISTING SYSTEM INTEGRATEDMEDICAL RECORDS MODULES PROPOSED SYSTEM DOCTOR Register Patient The users use the application to view his ailment and test reports. This module allows the user to upload external lab reports. The patient chooses his ailment and uploads the image file. The module allows the patient to upload any number of reports for an ailment. The module allows the doctor to register the patients. Every patient is associated with a unique id. His details such as Name , address and contact number are collected and saved to the database. The present scenario every hospital has their own way of maintaining records. Some record information in files and either give it to the patient or retains them in the hospital. Disadvantages: - All the documents are physically stored. Patient has to carry his record all the time. Possible loss of the file or damage to the documents. Identifying and searching records become complex. When large number of documents or more than one illness is associated with a patient then managing the documents become complex.
